REQUIREMENTS:
1. For Freshmen:
- Submission of SF9-Card with LRN and Principal Signature or PEPT/ALS Certificate of Rating
- A Certificate of Good Moral Character
- Photocopies of the PSA Birth Certificate and Baptismal Certificates
- A recent Chest x-ray result
- 2 pieces of 2x2 recent photos on a white background with a name tag
- A photocopy of the Marriage Certificate (for married female applicants)
2. For Transferees:
- A Certificate of Honorable Dismissal and Transfer Eligibility
- The Original Transcript of Records or True Copy of Grades
- A Certificate of Good Moral Character
- Photocopies of the PSA Birth Certificate and Baptismal Certificates
- A recent Chest x-ray result
- 2 pieces of 2*2 recent photos on a white background with a name tag
- A photocopy of the Marriage Certificate (for married female applicants)

We appreciate your interest! Application is made online. Below are the steps to guide you through the process.
Step 1: Online Application Form
Begin by filling out the Online Application Form available on our website. Ensure that all required fields are completed accurately.
Upon submission, your application, along with any accompanying documents, will be forwarded to the Admissions Office for review.
Step 2: Exam and Interview Schedule Notification
After reviewing your application, the Admissions Office will send you an SMS or email notification with the date and time of your scheduled examination and interview. Please ensure that your contact information is correct and up-to-date to avoid any communication issues.
Step 3: Payment of Reservation Fee
Once you have successfully completed the examination and interview, you are required to pay a Reservation Fee of PHP 500.00 to secure your place in the program.
This fee can be paid directly at the school cashier or through our designated payment channels for your convenience.
Please Note: The Reservation Fee is non-refundable. This fee secures your slot in the program and is an essential part of the application process.
